He is survived by: his wife Bridget Holiman Polk of Sheridan; his mother Anne Polk of Gainesville, Ala; his daughters, Holly Sory (Chris) of Bryant and Megan Polk; his stepchildren, Jaylynn Hutson (Blake) of Haskell and Drew Chitwood (Caitlynn) of Clearwater, Florida; his brothers, Bill Polk of Green Forest and Vince Polk of Gainesville, Ala; his grandchildren, Isabel, Barrett, Cora, Jasper, Murray, Witten, Blakelynn and Cooper; and his mother-in-law Edith Holiman of Benton.
