He is survived by: his mother Marion Marks (Perkins); his sisters, Jowanda Holden (Steven) and Janet Duchamp (Don); his sons, Jeffrey Lee Wortham, Jr. (Tambrie), Michael Wortham (Ashley), Kyle Wortham (Amanda), Cody Wortham and Justin Graddy; and his nieces and nephews, Ryan Shoemaker (Angel), Dakota Fletcher, Lacy Hodges and Jamie Fletcher.
