He was predeceased by: his parents, Normal Otto Sullivan and Mary Elizabeth Sullivan; his sisters, Norma Hocking and Elizabeth Sullivan; and his son-in-law Dennis Smithson.

He is survived by: his wife Shirley West Sullivan; and his sister Erma Hill of Pocahontas, AR. He is also survived by nieces and nephews.

In lieu of flowers, the family asks that donations be made to St. Jude's Children's Hospital.
