He is survived by: his children, Carolyn Mullins, Teresa (Gary Oels), John David Lewis, Deborah (Terry Mitchell), Phillip Wayne (Amber Lewis), Melissa (Brett Faulkner) and Dixie Lewis; his sister Bonnie Baskin; his grandchildren, Ashley, Cliffie, Katie, Cory and Kennan; and his great grandchildren, Maddie, Madi, Addie, Dani, Emma, Jase and Pyper.
