He was predeceased by: his parents, Albert Cook and Susie Cates Cook; his wife Ann Lea Cook (McDaniel); and his brother Donald J. Cook. He is survived by: his children, Susan Parish (Randy) and Alan Cook (Ginger); his grandchildren, Ryan Parish (Stacey), Andrea Tugwell (Dusty) and Jill Parish; and his great grandchild Jackson Leighton Tugwell of Midway.
