He was predeceased by: his wife Shirley Cook (Baker); and his sister-in-law Anna Lea Cook. He is survived by: his son Ray Cook (Nancy); his grandson Rustin Cook; and his brother Doyle Cook of Midway.
In lieu of flowers memorials can be made to the Merlene Etheridge fund that feeds youth on Wednesday nights at Midway Missionary Baptist Church 2961 Midway Road, Donaldson, AR 71941.
