He was predeceased by: his parent Jerry Sr. Hoffman; his daughter Kateria Hoffman; and his brother Jim Hoffman. He is survived by: his mother Ann Gossett Hoffman of Alma; his wife Rhonda; his sons, Trey Hoffman (Charlene) of St. Peters, MO and Travis Hoffman of Brooklyn, NY; his grandson Dereck Hoffman; his mother-in-law Alta Carpenter of Cherokee, KS; and his siblings, Terri Humphreys (Carl) of Alma, John Hoffman (Laura) of Alma, Jeff Hoffman (Susan) of Van Buren, Roger Hoffman (Libby) of Alma and Eddie Hoffman of Fort Smith.
