He is survived by: his wife Revis Chipman Blaylock of Manila; his children, Dale T. Blaylock (Lori) of Yorktown and Stacie Jo Booth (John) of Caraway; his brother Alton Blaylock (Bernice) of Dell; his grandchildren, Amber Booth, Morgan Booth, Malorie Booth, Brody Blaylock and Maesyn Blaylock; his mother-in-law Helen Chipman of Manila; and his siblings-in-law, Maureen Blaylock, John, Thornton, Joe, Pam Chipman and Dwight Booth. He is also survived by many nieces, nephews, and friends.
