He was predeceased by: his parents, Harvey Lee and Mary Elizabeth Ballard. He is survived by: his wife Terry Ballard; his children, Jerry Ballard (Sabrina) and Vanessa Wayne (Johnathan); his grandchildren, Kayven, Asher, Oliver, Aria and Johnathan; and his siblings, Harvey "Skipper" Ballard (Deborah) and Linda Alford (Brian). He is also survived by numerous nieces and nephews.
The family ask that everyone in attendance please wear a face covering and practice social distancing.
