He is survived by: his wife Virginia Cook; his children, Shirley Waterhouse (Mike) of Sherwood, Rebecca Smith (Rich) of Sherwood and Keith Cook (Beth) of Sherwood; and his brothers, William A. Cook and Donald Cook. He is also survived by 11 grandchildren; 18 great-grandchildren.
