He was predeceased by: his parents, Hunter Moore and Pauline Moore. He is survived by: his wife Shirley Moore; his sons, David Moore (Vera) and Eric Moore (Jessica); his grandchildren, Chelsea Hartwick (Jeremy), Stephanie Bittle (Tyler), Jackson Moore and Vivian Moore; and his great grandchildren, Jensen Hartwick and Olivia Hartwick. He is also survived by four siblings and several nieces and nephews.
In lieu of flowers, the family requests donations be made to St. Jude's Research Hospital or Healing Hands International on behalf of Jack.
