He is survived by: his girlfriend Teresa; his daughters, Shree Rainbolt (Ricky), Makali Altom and Morgan Howell; his grandchildren, Courtney Cook (Brandon), Jonathan Crossman, Alivia, Erica Rainbolt and Holden Cole; his great grandchildren, Braxton, Brenden Jackson, Lyla McInturff, Blakely Cade and Livie Cook; and his sisters, Diane Turner and Carol Ballard (Jerry).
