He is survived by: his wife Diane Teague; his children, Amber Howard of Lafayette, Sylvia Teague of Shreveport and Jamie Jacobs of Lafayette; his mother Charlotte Frazier of Sheridan; his stepfather Ronnie Frazier of Sheridan; his father Tommy Jacobs of Shreveport; his sisters, Mimi Jacobs of Fordyce and Nancy King of Watson Chapel; and his grandchildren, Brylee Howard, Jaid Howard, Elizabeth Jacobs and Isaiah Jacobs.
