He was predeceased by: his parents, Walter Berkholtz and Wanda Berkholtz; and his daughter Melissa Berkholtz. He is survived by: his wife Ethel Berkholtz of Highland; his daughters, Melody Folyer (John) of Horseshoe Bend and Cynthia Smith (Matt) of Metter; his grandchildren, Amanda Folyer, Jennifer Cunningham, Jessica Folyer, Ashley Kenny, Jasmine Smith, Mathew Smith, Triston Foster and Travis Smith; his great grandchildren, Elyxis Owens, Peyton Chambers, Makenzie Cunningham and Nolan Chambers; and his sisters, Joyce Loveridge and Judy Gaskins.
