She is survived by: her children, Duane Formby (Belinda), David Formby and Cheryl Banks (Gary Lee) of Patmos, Arkansas.; her daughter-in-law Vanassa Formby; her friend Mary Ann Flowers Tarpley; her nieces and nephews, Mark Honeycutt, Steve Formby, Suzanne Formby Marshall and Stacy Formby Taggart; her grandchildren, David Formby, Aimie Formby Barnett, Steve Formby, Jamie Formby, Amber Formby Stroud, Chelsey Banks Mills, Fallon Formby Scoggins and Chaley Banks Porter; her great grandchildren, Luke, Brooklyn, Colt, Jacob, Jessie, Madison, Westin, Maverick, Anna, Whitt, Knightley, Marshall, Jaxson, Everley, Maurie, Jagger, Jase, Mackston and Lawley; and her great great grandchild Sterling.
