He was predeceased by: his father John Chamberlain Sr.; his brother Thurman Alvarez; and his brother-in-law Cody Jordan. He is survived by: his mother Dorothy Chamberlain (Johnson) of Lowell; his wife Jessica Chamberlain; his children, Valerie Chamberlain of Hindsville, Katelyn Chamberlain of Gentry, MaKaila Chamberlain of Gentry and John Chamberlain 3rd; his siblings, Calvin Alvarez of Lowell, Crystal Chamberlain of Springdale, Sarah Phillips (Charles) of Springdale and Tina Chamberlain; his sister-in-law Vanessa Osburn (Daniel) of Elkins; and his parents-in-law, Randy and Sharon Jordan. He is also survived by many nieces and nephews.

In lieu of flowers the family request to make a donation to help pay funeral expenses.
