He was predeceased by: his mother Linda Ballard (Baldwin). He is survived by: his wife Judy Ballard; his father Pat Ballard, Sr. of Hickory Ridge; his children, James Pat Ballard, III (Jessica) of Hickory Ridge and Lori Ballard-Smith (Christopher) of Russellville; his stepson Michael Pruitt (Sarah Beth) of Cherry Valley; his parents-in-law, Desmond and Merna Murphy; and his grandchildren, J. P. Ballard, Jaden Ballard, Parker Pruitt and Raylan Pruitt. He is also survived by other relatives and friends.
