He was predeceased by: his parents, George Polacek and Mary Bruno Polacek; his brother Eugene Polacek (Vicki); his children, Kelly Dunn and Scott Polacek; and his grandchildren, Gretchen Ferderbar and Kaylee.
He is survived by: his wife Adrian Polacek; his children, Robin Peterson and April Henry; his grandchildren, Justin Henry, Payge Caulfield, Ryan Caulfield, Allysen Hollsten, Greysen Hollsten and Breyden Hollsten; his great grandchildren, Alex Marjie, Hannah Henry, Riley Henry, Delilah Henry and Braelyn Marques; and his brothers, Ronald Polacek (Maureen) and James Polacek (Sue). He is also survived by numerous nieces, nephews, other family and friends.
