He is survived by: his wife Mary Cook; his sons, Wayne Cook (Homero) and Randy Cook (Carolyn); his grandchildren, Justin Cook (Stephanie), Samuel Cook (Lori) and Emily Cook; his great grandchildren, Payton, Parker and Collin; and his siblings-in-law, Mike Martin (family: Larry Martin, Bob Martin) and Teresa Snow.
