He is survived by: his wife Lanora Elizabeth Mulliniks Hazelwood; his children, Kennith Calvin Hazelwood (Acacia) of Heber Springs and Rose Annie Burleson (Jerry) of Heber Springs; his siblings, Rhonda Hazelwood of Greers Ferry, Gizelle Smith (Perry) of Greers Ferry and Kyle Rufus Hazelwood of Greers Ferry; and his grandchildren, Alyssa Hazelwood, River Burleson, Skylar Burleson and Ezekiel Hazelwood. He is also survived by other relatives and many friends.
