He was predeceased by: his parents, Dave Lewis and Helen Cook; his brother Gary; his sister-in-law Cathy Cook; and his niece Kim Cook-Warner.
He is survived by: his wife Carolyn Cook; his daughters, Regina Ibbotson (Freddie), Tracie Harrison (Billy) and Angie Williams (Davy); his grandchildren, BJ Harrison (Chelsey), Josh Harrison, Taylor Ibbotson (Paige), Parker Ibbotson, Bryce Sims (Elaina), Brenon Williams (Abby), Brayden Williams and Brance Williams; his great grandchildren, Maeve, Bishop Harrison, Ellie Kate Ibbotson, Eden, Garner Sims and Palmer Williams; and his niece Carie Cook.
