He was predeceased by: his parents, Kennith Hazelwood and Ruth Hazelwood (Hooten); and his brother Craig Hazelwood. He is survived by: his sisters, Rhonda Hazelwood of Greers Ferry and Gizelle Smith (Perry) of Edgemont; and his sister-in-law Lanora Hazelwood of Wilburn. He is also survived by twenty six nieces and nephews, and many, many family, and friends.
