He is survived by: his mother Lana Sue Stark Fultz of Green Forest; his wife Joy Ann English Fultz; his sons, David Cohick (Virginia) of Green Forest and Joseph Cohick (Karen) of Ohio; his sister Janet Fultz Johnson (Eric) of Fayetteville, AR.; and his grandchild Jerry. He is also survived by numerous grandchildren and great grandchildren.
