He was predeceased by: his parents, Harold McCullah and Hazel McCullah; and his uncle Thomas Martin (Paula) of Farmington, Arkansas.. He is survived by: his wife Clare Hulseman McCullah of Farmington, Arkansas.; his sister Brenda McCullah Elkins (Larry) of West Fork; his niece Katie Rentsch; his nephew Kevin Elkins; his son Jack Bates (Anna) of Fayetteville; and his grandchildren, Dakota Bates (Vanessa) of Fayetteville, Levi Bates (Maddie) of Lafayette and Merritt Bates.
In lieu of flowers, the family requested donations to St. Jude's for cancer research.
