He is survived by: his wife Juanell Lum Smith; his children, Kay Middlebrooks, Ray Smith (Dusty) and Darlene Phillips (PeeWee); his grandchildren, Chris Middlebrooks (Lori), Craig Middlebrooks (Kristina), Eric Smith (Megan), Jonathan Smith (Angela), Anthony Smith (Amanda), Stephen Middlebrooks, Bridget Hatfield, Lacy Snow (Robbie), Camey Bridges and Brady Mathews; and his siblings, Rickey Smith (Debbie) and Teresa Gordon (Johnny). He is also survived by twelve great-grandchildren; two great-grandchildren on the way; and host of extended family and friends.
