He is survived by: his wife Betty Brewer Cook; his children, Jason Dan Cook (Carmella), Daniel Massie (Shelly), Nathan Arthur Cook, Reason Immanuel Phoenix (Nicole), Twila Angeline Cook Guerrieri (Jim) and Heather Vania Cook; his grandchildren, Kyle, Nicholas, Ryan, Destiny, Stormie, Dylan, Brittney and Kristin; and his brother Samuel MacArthur Cook.
