He was predeceased by: his parents, John Chester Polk, Jr. and Frances Vick Polk; and his sister Vickie L. Martin. He is survived by: his daughter Jessi Leigh Polk; his granddaughter Rebecca Lynn Gracie of Bryant, Arkansas; his sister Thrita Polk Griffith; his niece Kenda Chandler of Little Rock; and his great-nieces, Frances Thompson (Josh) of Lake Butler, FL and Brenda Chandler of Little Rock. He is also survived by five great-great nieces and four great-great nephews; and a host of family and friends.
Due to Covid-19 the Memorial Service will be postponed until July, the family will post the date and details then.
