He was predeceased by: his mother Ida Middlebrooks (Shull); his former wife Carolyn Hopkins Middlebrooks; and his parent L. G. Middlebrooks. He is survived by: his wife Darlene Middlebrooks (Ramsey) of Bradley, Arkansas; his son Jon Middlebrooks of Omaha, Nebraska; his grandsons, Christopher Middlebrooks (Andrea) of Baton Rouge, Louisiana and Alex Middlebrooks of Baton Rouge, Louisiana; and his great grandchild Parker Middlebrooks.

In lieu of flowers, donations can be made to the Bradley Methodist Church or the Bradley Food bank.
