He was predeceased by: his parents, Shirley I. Klevgaard and Orin K. Self; and his grandparents, Orval, Edith Self of Ada, George and Signora Klevgaard of Felton.

He is survived by: his wife Jeanine M. Clark; his sons, Jason Self of Bella Vista and Dr. Patrick Self (Caroline Muzic) of La Crosse, Wisconsin; his sisters, Brenda Andrews (Joe) of Bella Vista, Arkansas and Kim Moravec of Bella Vista, Arkansas; and his aunts, Brenda Cummings (Arlen) of Detroit Lakes and Kay Stevens (Ed) of Pace. He is also survived by many nieces, nephews, cousins, Air Guard family and dear friends.
