He was predeceased by: his parents, Harold Smith and Helen Smith; and his siblings, Harold, Bob, Don, Rick Smith, Barbara Peterman and Gloria Dugan.
He is survived by: his wife Hazel Thomas Smith; his children, Scott David Smith (Debbie) of Riverchase, Philip Joel Smith (Cyndi) of Wilsonville and Marni Allison Ulch (Kevin) of Vestavia Hills; his sister Martha Lee; and his grandchildren, Haden David Smith, Merritt Marie Smith, Philip Connor Smith, Andrew David Ulch and James Warren Ulch.
