She is survived by: her husband Phillip Ray Lewis of Uriah; her daughters, Heather Ward of Uriah and Mendy Nicole Crabtree of Uriah; her sisters, Theresa Jackson Belcher of Mobile and Terri Jackson of Dothan; and her grandchildren, Nathan Ward and Axton Macks.
