He was predeceased by: his parent Lee Forman; and his brother Mark Forman. He is survived by: his wife Vernell Shearer Forman; his brothers, Jeff (Nancy Forman) and Randy (Eileen Forman); his sister-in-law Avis Prince of Knoxville, TN; his nieces and nephews, Dale, Lisa Thomas of Scott, AR, Blake, Jennifer Prince of Knoxville, TN and Anna Forman of Chicago, IL; his great-nieces and great-nephews, Addison, Carter Prince of Knoxville, TN and Landon Forman of Chicago, IL; his godchildren, Mark, Rachel, Kelsey, Natalie Donnell of Mobile, AL, Brock, Mary Claire and Mimi Kelly of Saltillo, MS; and his friends, Jeff Spiller and Bob Donnell.
