He was predeceased by: his parents, John Cook and Annie Bell Cook; and his grandchild Chase Wilder. He is survived by: his wife Teresa Cook; his children, Tabitha Cook of Huntsville, Nikita James (Andrew) of Sylacauga and Trederick Cook of Sylacauga; and his siblings, John Cook Jr. of Toledo, OH., Herbert Cook (Mildred) of Detroit, MI, Dorothy Cook of Sylacauga, Sandra Whetstone of Detroit, MI and David Cook (Kathy) of Birmingham. He is also survived by four grandchildren; and a multitude of nieces, nephews, other relatives and friends.
