He was predeceased by: his parents, Jerry Kostelecky and Betty Lazek Kostelecky; his granddaughter Sarah Kathryn Burks; and his sister Charlotte Kostelecky. He is survived by: his wife Judith Sibley Kostelecky of Stapleton, AL; his children, Gavin Wade Kostelecky (Malinn) of Gulf Shores, AL and Laura Kostelecky Burks of Fairhope, AL; his brothers, Jerry Aurthur Kostelecky of Silverhill, AL and Robert John Kostelecky of Saraland, AL; and his grandchildren, Reagan Malinn Kostelecky and Gavin "Rhett" Kostelecky.
