He was predeceased by: his parents, Thompson Levertt and Bernice Boothe; and his siblings, Bobby Boothe, Donna Kay Johnston and Linda Roberts.
He is survived by: his wife Donna Boothe; his children, Kayla Carter (Jacob) and Kelsey Boothe; his grandchildren, Kaileigh and Eli Wayne; his siblings, Glenda Rada, John Wayne and Gordon Ray Boothe; and his mother-in-law Lillian Phillips. He is also survived by many nieces and nephews, other relatives and friends.
