He is survived by: his children, Janice Johnson, Jerri Nelson, John Cook (Kathy) and Jason Cook; and his grandchildren, Brandon, Ben, Allison, Geoffrey, Ashley and Hayley. He is also survived by 6 Great-grandchildren.
