He is survived by: his son Daniel Formby of Carrollton; his siblings, Lisa Wilson (John) and Jimmy Marshall; his niece Vanessa Nolan (Patrick); his great-nieces, Abby Nolan of Piedmont and Mary Nolan of Piedmont; and his grandchildren, Isaiah Lathan Formby and Annabelle Bree Formby.
