He was predeceased by: his parents, Jack Hazelwood and Mildred Hazelwood. He is survived by: his siblings, Ullis Hazelwood (Alice), Grant Hazelwood, Earl Hazelwood, John Hazelwood (Ann), June Lee (Ronnie), Frank Hazelwood (Sharon), Harold Hazelwood, Diane Hazelwood and Michael Hazelwood (Cindy). He is also survived by nieces and nephews.
