He is survived by: his son Cody Carlisle; his mother Susan Carlisle; his sister Windy C. Fomby (Chris); and his nieces and nephews, Josh Fomby, Brandon Fomby (Loren), Audrey Moran (Jordan) and Sarah Foley (Zack).
