He is survived by: his wife Jeanne Self; his children, Angela Agnew (Thomas), Kristin Elizabeth Self, Dwight Kristopher Self, Daniel Kevin Self and Jacqueline Ann Self; his mother Elizabeth Lynne Self; his siblings, Robert M. Self (Chas), James M. Self (Melissa), William T. Self (Terri) and Michelle Tiffin (Van); and his grandchildren, Camden Elise Agnew and Whitley Christine Agnew. He is also survived by nieces and nephews.
