She was predeceased by: her husband Bob Maddox; her parents, Robert Boyles and Connie Eubanks; and her siblings, Brent Boyles and Susan Prewitt.
She is survived by: her children, Amy Kelsey (Chris), Rob Maddox and David Maddox (Stephanie); and her siblings, Sandie Gaines, Darrell Eubanks and Robin Eubanks. She is also survived by her five grandchildren.
