He was predeceased by: his father Gordon Earl Puckett. He is survived by: his mother Peggy Sue Findley Puckett; his sister Susan Balkenbush (Steve); his niece Amy; and his great-nephew A. J.. He is also survived by several aunts and uncles as well as numerous cousins.
The family is requesting no flowers, and any donations be made in Michael's name to St. Jude's Children's Research Hospital.
