He was predeceased by: his former wife Nancy Kessler Hazelrig; his parents, Elige Hazelrig and Maebell Hazelrig; his granddaughter Allyson McCugh; and his son-in-law Scott McCugh.
He is survived by: his wife Freda Morris Hazelrig; his children, Phyllis McCugh, Carla Mayfield (Bennie) and Jack Hazelrig, Jr. (Marcia); his sister Linda Hazelrig Cabiness (Judy Hazelrig Smith); his grandchildren, Jack C. Hazelrig III (Jamie), Elizabeth Hazelrig Ethridge (Jason), Jon McCugh (Madison), Drew Mayfield (Danielle), Aleia M. Hyche (Ivy) and Cara M. Moore (Cody); and his great grandchildren, Sawyer, Lucas, Lige Hazelrig, Wyatt, Haze, Jacob Etheridge, Eva, James McCugh, Josey, Livie, Jacob Hyche and Cooper Moore.
