He is survived by: his wife Hazel Forbus Perry; his children, Karen Coogle, Sharon Worthy and Steven Charles Perry; his grandchildren, Natalie Worthy McMyler, Brittany Worthy, Callie Coogle and Michalyn Worthy Poole; his great grandchildren, Finley and Falyn McMyler; and his brother Luther Perry.
