She is survived by: her husband Eugene Polk (Gene); her sons, Randy Polk (Mary) and Jeff Polk; her grandsons, Stephen Polk (Brooke), Patrick Polk (Sarah Gray) and Andrew Polk (Ann); her great grandchildren, Caroline Polk, Hunter and Bryce Polk; and her nephew Scottie White.
