He is survived by: his children, Matt Cottingham (Stephanie) and Lori Polk (Brad); his daughter-in-law Amanda Cottingham; his grandchildren, Caitlyn, Morgan, Leah, Andrew, Emmy, Bennett, Giovonni, Sophie, Hadley, Sadie and Janie Kate; and his great grandchild Elli.
