With heavy hearts, we announce the death of Perry Chadwick Simmons (Hoover, Alabama), born in Owensboro, Kentucky, who passed away on February 27, 2018 at the age of 46. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He is survived by: his wife Cindy Keen Simmons; his daughters, Ann Marie and Caroline Simmons; his parents, Fred P. Simmons, Jr and Vivian Cripps (Frank); and his sister Allison Schrock (Bryan). He is also survived by nieces and nephews.

In lieu of flowers, memorial contributions may be made to the Ann Marie and Caroline Memorial Fund, in care of Wells Fargo Bank.
