He was predeceased by: his spouse Bobbie Sample; his parents, Russell Sample and Nellie Sample; and his sister Nell Vann. He is survived by: his children, Donny Sample (Allison), Kathy Wadsworth (Hal) and David Sample (Lisa); his grandchildren, Chris Wadsworth (Megan), Michael Wadsworth (Madison), Clay Jones and Abby Jones; and his great grandchildren, Camille Wadsworth, Mason Wadsworth and Daniel Wadsworth. He is also survived by four great-grandchildren.
