He was predeceased by: his mother Barbara Smith; his grandparents, Howard and Ruth Carroll; and his sisters, Teresa Pack and Beverly Self.
He is survived by: his wife Cynthia Smith; his children, Jeremy Smith, Austin Smith (Alyson) and Leah Allen (John); and his grandchildren, Brayden Smith, Grayson Smith, Easton Smith, John David Allen and John Brantley Smith.
