He is survived by: his mother Joy Perry; his wife Misty Perry; his sons, Chase Perry, Shane Perry and Ethan Perry; his father Michael Perry; his brother Jason Perry (Priscilla); his niece Kaitlyn Perry; and his mother-in-law Sherry Wood.
